Some people asked me why my blog tag line is “mencipta hari esok”, tomorrow already exist.

my answer is yes definitely god has created our tomorrow and future, but with his rahmah and kasih sayang he gives us many option of tomorrow.

And i always believe firman Allah in Al quran the “Allah tidak akan mengubah nasib sesuatu kaum itu selagi mana mereka tidak mengubah nasib kita sendiri”

So my friends, Tomorrow already existed, but why kinds of tomorow you are now choosing. Wheter You are comfortable for what are have right now or you want the better tomorrow that Allah has created. Choose it wisely

I believe it is not only tommorow. but we as a malay muslim, we have many obligation and responsibility on our shoulder that we should focus on. When the time come hopefully we are fully ready.

We cant change others, but can change ourselves. Keep our Doa and Allah will always listen and this is what he always promises.

Lead Part II

Part IX, factories and machinery (lead) regulation 1984 contain 8 regulations. Why this part is really special? Because it is specifically for the medical practitioner to know and it elaborate for about medical surveillance in lead.

In general it is a duty of the employer to institute medical surveillance programme for employees who are or maybe exposed above the action level (75 microgram/m3 of air) for more than 30 days per year, and the cost is on the employer expenses.

We have to remember this figures for blood lead monitoring :

  1. a) Period of 6 months,
  2. b) 3 months and
  3. c) Monthly

and the blood lead level range :

  1. a) 40 microgram/100 gram of blood,
  2. b) 60 microgram,
  3. c) 80 microgram and
  4. d) 73 microgram…why?

Because blood lead sampling and analysis must be done at least every 6 months to each employee exposed to lead as mention above, 3 monthly if the blood lead at or above 40 microgram/ 100 gram of whole blood but less than 60 microgram/100 gm of whole blood. And this monitoring must be done until 2 consecutive blood samples and analysis indicate blood lead level below 40 microgram/100 gm of whole blood.

The monitoring must be done monthly if the blood lead is at 60 microgram/ 100 gram of whole blood and the monitoring must be continue until two consecutive blood samples ana analysis indicate blood level below 60 microgram/ 100 gm of whole blood but above 40 microgram/100 gm or whole blood and once the blood lead has reached below 40 microgram/100 gram whole bood and the monitoring should follow the guide as stated before.

The monitoring must be done monthly during period of medical protection period and for a female employee of child bearing capacity J

Remember that this unit is lead measured in the blood not in the air (PEL).

If employee blood level make him eligible for medical removal protection, the employer shall provide a second follow up blood sampling test within two weeks after receive the results of first blood sampling test.

The sample of blood must be analysed by an approved laboratory, and must have accuracy to a confident level of 95 % with in a margin of plus or minus 6 microgram/100 gm which ever greater.

The employee should be notified if the blood lead level exceeds 40 microgram/100 rm whole blood.

Medical Examination

1) Annually when blood lead level is at or above 40 microgram/ 100 gm whole bloodat any time during the preceeding 12 months

2) Prior to assignment for the first time in which airborne concentrations of lead are at or above the action level

3) As soon as possible if the employee develop s&s of lead intoxication

4) As medically appropriate

And the medical examination should contained :

1) Detailed history and physical examination – occupational, smoking, lead related PE and History

2) blood pressure

3) Related Blood sample and analysis

4) UFEME

5) Any lab ix deemed necessary and shall include pregnancy test if necessary

What we must know about lead Part I (Permissible Exposure Limit)?

In this post I would like to highlight some important point regarding Lead specicifically for medical practitioner working in industry.

Adverse effects of lead to health are well known. It is a very good effort for government to enact factories and machinery (lead) regulations 1984 in Factories and machinery act 1967 (act 139).

This act contains 14 parts and 54 regulations.

The most important parts for medical practitioner who work under the umbrella of medical act 1947 are part II, IX, X and XIII

Part I contains 3 regulations mainly about ;

  1. Permissible exposure limit (PEL),
  2. PEL for exposure greater than eight hours and,
  3. PEL when Using the respirator

It is stated in regulation 5 that no employee should be exposed to lead a t concentration greater than one hundred and fifty micrograms per cubic meter of air (150 microgram/m3 ) average over an eight hour period

and in regulation 6 stated that if and employee is exposed to lead for more than eight hours in any work day, the PEL as a time weighted average for that day, shall be reduce according to following formula ;

PEL in microgram/mfor the day = 1200/hours worked in the day

In regulation 7, when respirators are used to supplement engineering and work practice control to comply with the PEL and all provisions of these regulations have been met, employee exposure, for the purpose, for the purpose of determining whether the employer has complied with PEL, may be considered to be at the level provided by the protection factor of the respirator for those periods the respirator is worn. These period may be averaged with exposure levels during periods when respirators are not worn to determine the employee’s daily TWA average

Generic or Original ? What Will Be Your Preferences? / Generic atau Original? Mana Satu Pilihan Anda?

Few years ago when I still practicing medicine in government hospital, some of the medications  are generic, it means that the medication is a copy of the original medication but have similar function.

As example the original  antibiotic such as Augmentin, it contains amoxycillin and clavulanic acid. Similar medication which contain similar drug as augmentin such as calamentin and many others are also available in market but suprisingly very much cheap compared with the original antibiotic with similar chemical properties and function.

The differences are the antibiotic are assembles locally or overseas, or using the other name with valid licenses.

My brain started  to consume the ATP since I was immediately start to think why we should use the original drug if the generic is much cheaper?

If you dont know  the price of Augementin compared to it’s generic, you can go to pharmacy and ask.

Recently I went to Sen Heng and plan to buy GoPro Hero 4 because I was planning for vacation. It Price rm 1600 and fortunately my friend also sell the generic sportcam as well.

So here some questions that I asked myself before I decide which one i should buy :

1- How frequent I will use the devices ?

2- How much i would like to burn my money and invest on that devices or gadgets ?

3- Is the generic one is relatively better? or more less like the generic one ?

4- What are the special functions that I really want in that devices ?  

and now guess…which one i already bought?

When You Are Depress

Recently I met few of my friend who became depressed may be due to financial, family, relationship or work related problems. When I write this post, it does not means that i never been depressed, I have experienced it a lot and i wish i never have to face this kind of problem again.

It is normal to become depressed as long as the depression not consumes you and your daily activities and your function not disrupted.

Stress not necessarily bad. because adequate dosage of stress will make us more productive. So, what we need to do is to titrate the amount of stress so it is just enough to make us more productive and not debilitates you.

Engaged the depression before it it start to hunt you. Detect it early and you definitely can prevent it.

Anyway Selamat menyambut Ramadhan dan Tak lupa untuk Kita sama doa2kan saudara kita di Sabah.

10 Percent of Your Income

Ever heard about the rule that you must save 10 percent of your income? For me personally, i would like to save more than that and as much as possible

maybe some of has difficulty to abide this law because of financial constrain. Some of us only has small income or maybe having to many other commitment such as personal loan, housing loan and car loan.

how about if we can reduce our monthly spending for 10% only and the money goes to saving ?
Or make another source of income equal to 10% that you must save.

Or if it is really difficult enough how about cutting your monthly expenses only 5% and get extra 5 % income to fulfill the 10% rule.

I believe everyone can do it. But we need a little bit more of discipline

Mutiple Streams of Income

I have very little knowledge about money. But what I would like share might be important for someone of you who just graduated and still young. I just aware about this things when I already quite old. So I really hope with this little knowledge you wont repeat my previous regretful past = being financially idiot.

Why actually we need to have multiple streams of income?

1- Price of goods is not static and has no empathy for your financial situation

Every years prices of every goods and services will definitely raise even though the price of oil is dropping. You can accuse any orang tengah or dealer whether they are Chinese or Malay but believe me, it is really difficult for the price of goods to drop.

So with multiple stream of incomes, you can actually combat this situation and not only treading the water for the rest of your life. Instead you can fly high. ever heard about financial freedom? We will talk about it later in another post

lets say that your monthly income is RM 3k for fresh graduate. Do you think that this RM 3k is enough? Believe me as the time progresses, with the inflation and after you get married and have a child, with this amount of money you will initially start to have deficit in your budget and eventually you will sink in ocean of financial crisis.

So before that happen never depend on single income or job. Make another source of money

2- At time of crisis

Let say economical crisis just like in the  year of 1997-1998 suddenly emerge in front of your door. And you get fired because your the company that you work for are going for bankruptcy.

Definitely this will affect you but the damage can be minimize and you can fully concentrate on you other source of income.

But remember to do in one bye one without feeling of self complacency.

3- Help the others

I know your are a good guy. But please, instead give a man a fish, teach him how to fish.  Make your own business and hire the others. And teach them the same thing
